I am new to New York and need some alterations done to a few suits and a couple pairs of pants.

Can anyone recommend someone reasonable but who I can trust working on a few pricier items (ie not your neighborhood dry cleaners but I don't want to spend more on the alterations than the clothes themselves)?

I know I'm beginning to sound like a broken record talking about this guy, but Alexis at 242 East 77th did a good job for me. Not cheap, I dont think ($200 for taking in the waist on the jacket, shortening sleeves, fixing pants waist and cuffs) but good.

Did the waist suppression or sleeve work entail adjusting the shoulders?

I was going to try him before NSM started making trips to NYC 2 years ago. The sleevehead blog is the only place I've heard of him, sleevehead reads/posts here (not sure of his SF name) so maybe he can give some more color as he has also tried NSM.

In terms of Raphael, I think there is only one and I don't think he is making suits anymore but I could be mistaken, a few people here are customers so maybe they'll chime in.

Has anybody ever heard of Carmine DelGuidice? He used to be on 57th for ages, but recently (a few years ago) moved next door to my parents in Jersey. He still cuts and tailors all himself at his house. I saw his workroom. Looks nice.

the same company owns TJ, Oxxford, Individualized shirts, Holland and Sherry too I think. Anyway. you are asking for very different things when yuo are asking about Tom James, NSM/Neducci. You sound like you need to be edumacated on different house styles, difference between bespoke/custom/mtm and various costs. There is a lot of info on NSm on here and probably Tom James if you search. That will lead you to discusions on other tailors and styles.

thnx guys. yeh im gonna try out a few different places....i range from the keep the suit in the office in case u have to fly on short notice to the good stuff u keep in the closet for important events etc...we'll see where i end up
