it's for orthopedic reasons. very common in the austro-hungarian bespoke tradition...
That's what's known as a Thomas heel. It is supposed to prevent pronation--walking the shoe to the inside (medial side). It doesn't always have that severe a "corner" on the lateral side. That seems a bit unstable to me but I could be wrong.
