Several years back a man was found dead in a Tijuana hotel room from complications (gangrene I think ) resulting from the amputation of one of his legs. i think the San Diego police got involved and it was a real puzzler at first. Whats this american guy from the midwest somewhere doing down here? Where did his freshly removed leg go? etc. Turns out he had decided to realize a lifelong dream (I think he was late middle age) of having one of his legs removed. As you might expect, practicing stateside Drs told him to **** off so he got a grossly alcoholic "former" dr to do the deed in a TJ hotel. There is an actual scientific name for this desire. As you might have guessed it is quite rare.

In American culture (at least), we have pretty disordered relations to the fact and the ways that we're embodied.

As I vaguely understand, there are some Indian religious festivals that practice "hook-swinging" whereby the people are hung by hooks and swing about. I think it was banned under the British though.
