Ugh, the $/£100 return expense is what's holding me back. After waiting for 6 months I'd be sad to receive a pair that didn't fit.
On the other hand, the only shoes that have been real fitting fails are the two that I purchased in store with a sales rep; I have a pair of Aldens that are too narrow despite being sized on a Brannock device and recommended a 8C on Aberdeen last. There's also a pair of Carminas that gap at the sides as it was maybe for a lower/higher instep on Costix last.
So there's a easy $1000+ spent on two shoes that I can't wear. I will eventually get around to posting them on eBay or maybe I'll try posting here on SF and try to get some of my money back.
Whenever I've ordered a standard US10 or sized online, they've tended to fit well. This includes Crockett & Jones Sydney loafers and Cheaney monk straps in UK9.
I did the ABC measurements so I'm 90% confident I'm a 9.5 in Meccariello.
I would suggest you to buy a pair of his new Roma line shoes in Chisel 4, since UK 9.5E. Chisel lasts have a rather standard fit in terms of length, width and instep within his collection, and the new Roma handwelted line is an exciting new development in terms of a lower cost alternative to traditionally fully handwelted and fully-sewn outsole. Its construction is similar to Saint Crispin's so they are still very good shoes - especially given the price.
At around EUR 480 and without the need to wait 6 months, this should be a quick way to confirm the fit and correct size. You could even ask Antonio to do last modifications based on the issues you find on the Roma pair. It is basically a wearable pair of fitting shoes.
Then of course, I am persuading you to buy it so we can have a look at them when you unbox them - that's the genuine reason behind my logical arguments.
I'm curious would he have dark brown suede cap toe? and if chisel 2 bleeds top of my thumbs, what to do with chisel 4?
You mean something like this?
Equites 2 Captoe Oxford in Dark Brown Suede
Handwelted Equites 2 Captoe Oxford in Dark Brown Reverse Baby Calf Suede. Made on the Argentum Soft Square last with Oak Bark Tanned Soles. For out of stock sizes, pre-orders takes between 12-18 weeks.www.meccarielloshoes.it
And the Argentum Round last helps a bit
he has a page of other shoe's size vs. AM's various last, did you try that as well?
two things to watch out are width and instep. So I take generally you're not narrow or wide width (though if SA ask you to take narrow in Alden Aberdeen which is already narrow to start with you might be narrow but you did say it's too narrow?), instep on any AM is going to be higher than any English.
that's not Roma! and yes I saw this but also can't figure out what to do about size.. some people size up from chisel I recall..and anyway it's a "want" and not "need" so it's not rush...

Don't they look good as fitting shoes?
It’ll be interesting to see what Antonio comes up with for the make ups. I think the black punched cap toe is available now and then the adelaide will be available in two colors toward the end of the year. This is the first I heard about the Roma line. I looked it up on his Instagram and that's great. I would be interested to get a pair of punched cap toe oxfords in the dark brown caffe tostado leather.
I think the Aberdeen last was just too narrow for me even at a size 8E (8.5 was too long)...I think I mentioned it, but thought that it might improve with wear. Frankly I think the SA should have guided me to a different last. So I think I'm a standard D width in US sizing and generally a size 10.
I think the Costix last might have a different instep. It's the only shoe I have that has the gaping issue and I feel relatively confident that I have a "standard" instep.
I guess I only have issues with lasts that are perhaps ill suited for me, and this actually makes me a bit more confident. I will try to connect with AM on Instagram as suggested.
I have two pairs of Carmina oxfords, both on the Rain last and both the same size. One gapes, one doesn't. Go figure. The gaping issue could be just due to having flat/semi-flat feet. I have this problem on some shoes too and have since found out I have a somewhat fallen arch. Vladimir Riche has a very good video on Youtube about it.
