Roomier dress shoes to fit orthotics?

For those who wear orthotics, do you wear roomier dress shoes (higher instep + wider width) to fit orthotics or do you simply forgo orthotics altogether?

Orthotics that are sized correctly shouldn't require wider shoes. Orthotics probably won't fit unless the shoes have insoles that can be removed. Judging by the collecting habits of Style Forum members, not many regulars would skip or tear up shoes from the majority not made to easily accept orthotics.

It really depends on the shape of the orthotic and the shape of the shoe. I know that Alden, for example, makes some styles designed to accommodate larger orthotics. I'd guess that Allen Edmonds does as well. If you're wearing custom orthotics, whoever is fitting them can probably give you some advice. If you're on your own, you'll have to look for

I don't know where you're located, but I highly recommend Moulded Shoe in NY. They are expert fitters.
