Outer rear heel, and inner toe. Toe goes first. I use taps (okay, hammered in cut tacks- an idea from a thread here on SF) to address the toes, and I have the heels addressed as needed. They don't usually go too fast, and aren't pricey to have fixed up.

Just a note to clarify things...

"Gait" is the way in which you walk, not the wear patterns on the shoe that result from that manner of walking.

And in passing, I was always taught (don't have much in the way of evidence) that wear patterns differ among ethnic groups. For instance, it is common and considered normal for people in western cultures to wear the outside corner of the heel first and foremost (this I've seen and agree with). Wear at any other place on the heel usually indicates a need for corrective measures.

Wear in the center of the tread area, with maybe a slightly medial aspect, on the outsole is considered normal. Excessive wear at the periphery or along the edges is also indicative of an abnormal gait and may require corrective measures.

That said, improper balancing of the shoe when it is bottomed can also affect these wear patterns, as can improper fit...and sometimes even lead to debilitating physiological changes in the foot structure.
