Web survey tools for staff feedback

Im looking for a good web based survey tool to use to gather feedback from my staff on my performance.

I am used to getting performance feedback however since moving into a management position it has been lacking - i plan on remediating this by setting up my own survey for my direct reports and their staff. I have had a look at a few survey web sites but they all seem limtied or over priced. I need the following functionality

- Anonymous login for staff
- Free or cheap (one off payment, i dont want to pay a monthly fee as im unlikely to use it more than twice a year)
- Ability to ask both multiple choice questions as well as reply to questions in essay format (for additional feedback)
- Be fairly proffessional - no annoying popups or anything, i expect the staff to use this during work hours.

In addition to this im happy for any feedback on ways to format questions to get the most honest feedback (ie avoid leading questions, provide the correct spread of response choices in multiple choices etc)

Use google forms to make a free survey. No one uses survey monkey anymore since the free ver is so gimped.

survey monkey plans are too expensive. i don't want to spend $250 to raise one survey.

google forms seems to be what i need - thanks.
