Yup...outfit looks fine to me. I don't see the problem with low-rise jeans that reveal the belt. That's a look...and a very common one in a lot of fashion mags both in the US and in Japan.

I believe that he is stating that you do not leave it in your mouth.

Regarding use of the toothpick I was taught that you either excuse yourself from the table (and go do it in the bathroom or wait until you're in your car or something; hence why toothpicks are usually located at the exit and not given to you at the table) or you cover your mouth with your other hand while picking your teeth. The first option is preferable.
