Washing OCBDs and other cotton garments

Greetings all,
Apologies for the rather dull and oft-discussed topic, but could i please gather member's thoughts on how best to wash an Oxford shirt, to prevent shrinkage/fade/lost buttons/collar/lapel-ruffling, etc?
i) unbutton the collar and sleeves?
ii) inside out?
iii) 30C cotton setting, 1600 spin?
iv) rack dry, hanging, and no tumble dry?
Anything else?
I haven't always aheared, and often see shrinkage and a loss of buttons.

Even more irritating, on t-shirts, such those made of softer Sea Island Cotton (e.g. Smedley) i often see a disconcerting narrowing and shortening of the hem. (see attached)
How can i avoid this?

I don't do anything too intense with my laundry generally. For anything that doesn't need to be dry cleaned or hand-washed: I was on cold with gentle detergent. Hang dry. Steam or iron as needed.

That's what I do for basically everything ("dress" shirts, oxfords, flannels, linen shirts, cotton chinos, jeans, tees, etc.) and I rarely have any problems.

Oh and I wash whites separately, sometimes with Oxiclean to brighten.
