Jantzen measurements question

I'm planning to make my first Jantzen order, and I need advice on how to specify correct shoulder measurements.

Jantzen classifies shoulders as square, normal, slope or very slope. I recently had a Samuelsohn MTM (45-38 Long) done up that fits perfect. The tag in the jacket reads "256 SLOPING SHOULDERS 1/2" and "301 RIGHT LOW 3/4". My right shoulder is lower than my left shoulder (and my right arm correspondingly longer than my left).

Can anyone tell me how to translate these measurements into a Jantzen shirt order?

on the order form ask for the blue or yellow shoulder line.
then tell them that your right shoulder is 3/4 inch lower than the left.
then give two different sleeve lengths.

I was just at Jantzen yesterday. Ricky corrected my initial order from "sloped" to "very sloped". He said the problem could have been avoided if I'd included a photo of myself with the initial order. So my advice is to send him pics of yourself along with your order.
