How tight should shirt cuffs be?

Depends on the watch. I like the slim cuff look, but it eliminates the chunky watches that many here are so fond of.

When the cuff is buttoned i can comfortably put one finger between the cuff and wrist. No bigger , no smaller.

In general, I prefer tighter cuffs.

I have the cuff button(s) moved such that the cuff ends 1/2 inch beneath my wrist, the latter where my coats are tailored to end. I measure and mark my wrists with an ink pin so there's no ambiguity for the tailor.

The results appear like Mafoo's in his picture, and I get consistency when mixing shirts and coats.

For bespoke, where it falls on your hand should not be based on cuff circumference when buttoned, but based on how the sleeve is cut and set. Whatever is a comfortable circumference in relations to accessories and jacket's sleeves will do.

For others, at the mininum one fingerbreadth (3/4 " or 2 cm) between cuff and wrist as Despos mentioned, but tightness for me is determined more by how I want the hand to be "framed" by the cuff (i.e., where it falls on the wrist/hand).

I like them quite slim, otherwise they ride up if my jacket moves or they fall too low (assuming the sleeves have a bit extra length) and I hate the feeling of them sitting on my wrists/hands. I also have my left slightly larger then my right to accomodate a watch. I think the 1 finger sounds about right.
