I am a 14/36 dress shirt size (yes, you read that correctly) -- am I doomed as far as OTR dress shirts go?

I've spent most of my life buying dress shirts off the rack and they've never fit me, at all. I have a pretty small neck and pretty long arms (14/36) and so most of my OTR shirts are 15/33s. They never poke out beneath my suit jacket and are always a bit roomy in the neck.

Are there any OTR options that would accommodate me or am I gonna be ordering from Proper Cloth if I want something that fits me?

Usually I'd suggest looking at tall ranges/shops for the extra arm length (even if you're not that tall, the extra body length's only a big problem if you want to wear a casual shirt untucked), but the neck size could still be an issue. (I have the opposite problem: a wider neck than someone with my chest size 'should' have.) It sounds as though you probably want a long size below M (assuming it would fit your chest) and that would rule out places like Lands' End and probably even a specialist like 2tall. Girav might be worth investigating but they do sleeve measurements from the shoulder, so it's hard to be sure.

Not a big deal. Order whatever size and fit you want from Charles Tyrwhitt. Select Shirts, then Custom Shirts. Not expensive, imo.

That's true, there are places that offer custom sleeve lengths without being as complex as Proper Cloth and other MTM options; I didn't mention any of them because the OP asked for OTR and something like CT custom is still (simple) MTO.

The viability will depend on how slim the OP's neck is relative to his chest: CT's system lets you pick from different fits but they're all designed around the assumption that a 14" neck corresponds to a chest of 34" max.

My neck actually generally matches up with my chest and midsection -- meaning, I have a somewhat small neck and a somewhat small chest and torso. Size 14s/14.5s generally fit me in the body well with sleeves that are up near my forearms. So the places allow you to choose neck and sleeve size only might actually work for me.

Sidenote, I feel like I don't have a crazy unusual body and yet my size seems so out of the OTR norm... I'm 5'11" 170 lbs. My arm length seems pretty normal for my height, maybe slightly long (36) and my neck seems fairly normal for a thinner person (14.5 but can fit in a 14). What gives with the limited selection of OTR shirt sizes?
