Shirt fit?

I'm thinking of getting a few shirts from MT and am thinking of using this shirt I had custom-made a few years ago:

The neck is a bit tight so I'll probably have them go up 1/4". Other than that, any obvious problems??

It looks tight all over, and the shoulder slope is screwed up. I'm not sure if MT can deal with that or not.

Oooh....that bad, huh? I have to admit I did gain "some" weight over the past few years.

FWIW, here's what it looks like tucked in. Can I use this as a basis for the MT shirts or do I go through my closet and find something better?

there are lots of things wrong.
have shirts made by someone who will measure you

it looks sloppy sleeves are too long, cuffs too big sleeves to wide, armhole too deep chest is a bit large as well
otherwise it is great
