It's almost always intended to mean "every two weeks."

While it can be used both ways, IME, most people use it for every two weeks. . . e.g. biweekly pay periods.

Actually, the first definition in Webster's is twice a week. Every other week is better said as fortnightly.

If we extrapolate somewhat to some more commonly-used terms...

Semi-annually = Twice a year
Bi-centenially = Every 200 years

So I voted for "every two weeks." That, and the whole biweekly pay thing.
