Somewhat related to the topic, there is a documentary on Netflix Instant called 'Snuff' which is about whether or not a snuff film exists. It is not particularly well made, and the people interviewed looked like they all ride the same smalltown bus to work, but it covers films like 'Cannibal Holocaust', 'Faces of Death' and the like.

I guess the Russian beheading film is the closest to meeting the definition although it seems to lack the profit motive.

When I was a sheltered early teen (or maybe pre-teen), I watched Candyman and it made me scared to go into a dark bathroom or stand in front of a dark mirror for years afterward... okay... i still don't like to go into a bathroom in the dark and stand in front of the mirror.

Now, I realize it was the creepy Philip Glass music as much as the movie itself... but anyway...

Tras el Cristal/In a Glass Cage is a favorite of mine in this category.
I love gore films like Hostel, etc., but don't consider them "disturbing."
Visitor Q is overrated, but gets points for being unique (especially if you like lactating breasts).
Salo or the 120 Days of Sodom is great. It's available from Criterion again. I think I'll put that one on my Christmas list and shock my grandparents. The disc I bought many years ago turned out to be a fake.
I liked Mysterious Skin (although not really disturbing other than the fact that child molestation is disturbing), but the whole alien references were portrayed poorly. I understand their importance to the story, but there's a silliness about them and the acting of those parts of the film, that brought the whole film down a level.
Requiem for a Dream... more for high schoolers trying to make the step up from films like Armageddon.

I thought Children of Men was disturbing just because the atmosphere and the future of the world that Alfonso Cuaron created (with the help of the original book and other influences) was so realistic.

Another film that particularly weirded me out was The Tracey Fragments starring Ellen Page, the art direction was way too trippy. The whole movie didn't really make sense but definitely odd to watch.
