I won't post all of the obvious (classic) ones right away, but a couple favorites that I feel are drastically underrated. What are some of your favorites?

The Decemberists Hazards of Love was IMO one of the top 5 albums of 2009. Saw them live at Lollapalooza and they killed the performance. Played the album front to back amidst the perfect dreary/rainy atmosphere.

One of my favorite hip-hop concept albums is NightHawks (Cage and Camu Tao). This is definitely worth a listen if you like underground hip hop at all.

The One Giveth and The Count Taketh Away!-Bootsy Collins
This one sucked so badly that I became instantly angry and I sold it to a used record shop after the first listening.

The obvious:

Queens of the Stone Age- Songs for the Deaf
Pink Floyd- Dark Side of the Moon
Sigur Ros- ( )
Radiohead- OK Computer

Less obvious:

Failure- Fantastic Planet
The Thrills- So Much For The City
Beck- Guero
Lovage- Music To Make Love To Your Old Lady To
Midlake- Trials of Van Occupanther
Underoath- Define the Great Line

Not sure if Tool would fit under "concept" since they constantly do "concept" albums but Lateralus is my personal favorite.

The 70's was the golden age of the concept album yeah?
Rush 2112
Alan Parsons did Edgar Alan Poe, some good stuff in there
Rick Wakeman - everything inc Henry VIII, Journey to Cent Earth, Camelot or some such
Elton John - Captain Fantastic, Yellow Brick Road
Pink Floyd - Dark Side and Wish You were here
Bowie - Ziggy , Diamond Dogs

course a lot of people say it's all too cerebral - classically trained rock musos trying to get serous and compete with the big boys of orchestral music and esp Opera,
my fav would be Dark Side
