Yeah, I was surprised by how high and blobby a toe box the Darltons had when I first got a pair (bought online, so didn't see in person). I learned to love them, but it was not love at first sight. They do not photograph representatively on e-commerce sites.

No, I actually did not do that drawing. But that drawing put me in the right direction, after I had tried it previously in a wrong way.

Take a sheet of paper (A4), fold it in half, draw half your design (every medallion has a centre line, which is your fold line) using a compass and 'French curves'. Once you're happy with the design, use a pair of dividers (set to a distance) and mark the centre of the holes (you might have to alter your design to get the spaces even). Once you are happy, use an awl and prick your design to the underside of the folded paper. Fold out: now you have your design. Draw all the prick marks with crosses, indicating the centre of the hole to be punched.

Use the photocopier to scale the design down, until you've reached the right size. The size of the holes is determined by the size of the punches. Every shoemaker/closer will have a large selection in various sizes. You only need to mark the centre of the hole.

I think different medallions may look better or worse with different styles of shoe.

This one looks good to me on a wing cap:

because the diamond at the top sits nicely with the peak of the wing, while the sides follow the shape of the wing.

This one looks good on a straight cap:

because its triangular shape emphasises the shape of the cap.

My favourite, the ram's horn, is relatively simple, but I think it looks good with both wing caps and straight caps:

Way too late to worry about it. From an SEO and corporate firewall standpoint, we've probably got enough threads with "Appreciation" or "pr0n" in the title to get blocked as a pornography site by many big companies.

I own a pair of Selbournes. They were my regular black leather-soled office shoe for a few months last year. I'm pretty sure nobody even noticed that there was a medallion on them, let alone that it could be viewed as being vulgar. (I certainly never thought of it myself until I saw people making cracks about the design around here.)
