The 46 references one's chest
The purpose is to measure the suit itself to find out the actual size of garment (not the wearer).
Can I take in the sides of this suit jacket? everything else fits perfectly. The tailor that altered the suit for me says she can't take in the sides anymore without ruining the fit.
In my opinion the suit doesn't lend itself to be taken in that much. It'll ruin the proportions of the jacket because the arms are too wide for such a tight cut. They are also a bit long, which you might want to address regardless of whether you have it tightened a bit or not.
