How much can you alter a jacket with working button holes without cutting from the shoulder? Can it be altered that way at all?

Very little can be removed or added at the sleeve's cuff-end without completely destroying the balance and proportion from bottom-most button to sleeve-end. Perhaps one-quarter inch at most is possible IN GENERAL.

Shortening from the shoulder is possible if you trust your tailor and want to invest (on average) $125.00 per sleeve. The jacket should have some sort of value in your mind for an investment in this procedure.

Lengthening from the shoulder? Forget it! It's highly unlikely! Find another jacket!

Assuming you want to lengthen the sleeve:

If all the buttons are functional, couldn't you add a fifth button? It may look better than the extra space.

And if only a few of them are functional (apparently somewhat common on English bespoke), I imagine the tailor could undo the fake buttonhole and add another at the bottom.
