So I am having sizing issues with shirts. It is because of my big shoulders I think. I have a rather thing waist, and like wearing pretty slim shirts, but I think I have bigger than average shoulders. So If I buy an XS from J.Crew for example, it fits my torso fine, but it pulls up at the second button from the top and I can't even clip the collar (I guess I have a big neck too).
Anyways, I can make a small work, but in general a Medium fits my shoulders and neck the best. By then though, the shirt becomes a tent. (The slim version of their small has the same pulling problem, and the slim M is still to baggy).
Anyways, my question is, if I take an M sized shirt into a tailor and have them take it in, is it even possible for them to make the shirt fit how I want it to, or is it physically impossible? (I dont really understand how shirt fitting works that well). If they slim it up how I want will it just pull like the normal XS and S? Also, does anyone know about how much it would cost to get the torso slimmed?

PS: actually, oddly enough, the pulling looks quite a bit like whats going on in this advertisement.
BTW I am 21yo 170cm 57kg so I'm not sure, but I don't think losing weight would help
