where to start making clothing?

I've been working some on sewing by tailoring my own shirts at home, and I'm interested in perhaps trying to actually make something from scratch.... but I have absolutely NO idea where to start. What kind of garment should i start with? Where do i get decent patterns? (should i even USE a pattern?) Where do I obtain decent fabric?

Lotsa questions... really i just have no clue about this stuff and have no idea where to look or start.

A shirt is a good way to start getting into it.
Don't worry about patterns too much for now. Make a shirt first, then let's talk about making a good shirt.

I no nothing about this, but my first thought would be to take a shirt and slowly deconstruct it, then lay out the pieces, cut patterns to match, then use the patterns to make a new shirt.
Start there.

Go to www.cutterandtailor.com for instructions for drafting patterns, or buy a book such as one of those by Winifred Aldrich, then get David Page Coffin's book on shirtmaking to learn how to construct a shirt. If you want to move on to tailoring, then the Cutter And Tailor Forum combined with a book on tailoring such as those by Hostek, Cabrera or Whife should contain all the information you need.
