Sorry, got them from the TM Lewin site. Guess they call them that to make it seem more British
They translate into:

medium spread
modified spread 2.5'' points
extra wide spread 3'' points
button-downs

I don't wear button down. I did when I was in high school. I don't mind it on others, its just not my favourite.

No point. Medium to wide spread. Larger size preferable, but not enormous.
