Is this nightmare from the Sartorialist?

Notice the sleeve cap on his jacket. This is what is called a dramatically roped shoulder. A very Southern Italian detail and a very acquired taste.

How do you get that much puckering everywhere? It looks like he has a wadded up pillow inside the sleeve, and like he did some odd thing to the chest pocket to make it look like my 80 year old aunt's face.
