Use more padding on the lower side to ensure symmetry or leave the unevenness and have the rest of the jacket adjusted to ensure proper balance?

    Or are other options?

Not if you want each shoulder height and angle to look identical. But that is not really that important. Unless your drop on one side is really huge, hardly anyone will notice.

The important thing is to make sure the suit fits properly in spite of the drop, i.e., that the buttons and buttonholes line up, and the droped side does not hang down too far.

The effect of a dropped shoulder will be to throw off the balance of the suit. The vertical line down the center of your chest will be affected, as will the horizontal line at the bottom of the jacket. Because the whole thing hinges on a button or buttons in the middle that must line up properly with holes on the other side, a good custom tailor needs to consider all these variables before he cuts.
