Tailored Shirt Divoting

I just received my first Online MTM shirt. While the fit seems good, and I'm happy with the craftsmanship, there is a small problem that the shirt, at the shoulder, produces a divot when I raise my arm.

Is this a function of the shoulders being too wide? The armhole being too small or too large? How should I change my measurements to eliminate this in the future?

The shoulder seams go to the edge of my shoulder, where the arm just begins to slope down. This means they are slanted from the armpit to the edge. Should they be more vertical (hence a smaller shoulder measurement) to eliminate this?

It's a function of the yoke and the armhole seam being much stiffer than the sleeve. Wash the shirt a couple times and see if it helps.

It is not an issue. Sleeves are at least 1 1/2" too long. Body fits well

Also, dress shirts aren't made to be able to do a full on backstroke in them.

Perhaps, but I think you should be able to do a karate chop or hail a cab without the menace of the divot.

I think that Carl should design a shirt pattern made specifically for men in a seated position, elbows at sides, fingers on keyboards...perhaps incorporate some clever pleating at the elbows and provisions for one-handed typing for those who post frequently on the Current Events subforvm.
