I'm a 40-42 and don't know if I need a M or L, so I'm curious if I need to size up or if these are TTS.

Depends how you want to wear it. I am a XL in their stuff. Usually a 44-46. If you want a slightly snug fit and do not plan on wearing a thick sweater underneath go with a M. If you want a looser more casual fit or want to have a heavy layer on underneath go with the L. I went with the XL for a more tailored look.

it was brought up a while back but has anyone recently grabbed any dress shirts from here? how is the quality?

Good quality, yes. High arm hole but straight through the body...really not that slim but very comfortable. Collar is designed to be moderately stiff so it will stand up without a tie. Can easily be worn as a dress or sport shirt.

I'm pretty sure all the SM label made in England shoes are AS.
