Is the top piece (the piece touching the ground) on the heel glued on, on quality shoes? Or is it attached with nails? If a cobbler later glues on a top piece, is it easy to remove and replace the glued on top piece?

Can be both.

If a cobbler later glues on a top piece, is it easy to remove and replace the glued on top piece?
Yes for straight rubber insert.
No for dovetail insert,
take more skill.

Yes, it's easily removed (with a bit of force). The "˜top lift' is normally a single piece (those leather and rubber ones come pre-assembled).

The shoe repairer rips off the old one, fixes the new one (slightly oversized) and sands it down to the correct size of the heel.
