In any of Flussers books does he ever give a definition of MTM and Custom/Bespoke and how they differ?

IIRC, whichever one is 50% city by city shopping guide (Dressing the Man? Clothes and the Man?) has a definition of made to measure vs custom.

m2m: you pick the style and the cloth. the store tailor measures you and the order goes to the factory. there the suit is made on anassembly line just like rtw garments.
then the finished suit is sent to the store. the garment is made on the same assembly line that the rtw garments are made. the store has to fix the details.

custom: you pick the style and cloth the tailor measures you and makes your own personal pattern. a raw basted fitting is made and fitted on you the tailor makes changes. another fitting is made with the garment partly done and it is fitted on you. if the tailor is not satisfied he may have another fitting. this can go on until he is happy. sometimes it may take only one fitting. the garment is not done until the tailor is pleased with his work of art.
