Proper Shoulder Measurements/Style for Sloped Shoulders

This is likely a newbie question, but I have two questions regarding shoulder measurements for suits. When I purchase suits in stores, sales reps tend to cite my shoulders as the biggest problem in finding the right fit. I was wondering if anyone could offer some suggestions to help me be a little more informed sport coat shopper.

1) When you take your own shoulder measurements , is it ideal to find a blazer with a near matching measurement? I.E should 19" shoulders correspond to an 19" shoulder blazer or should you allow yourself a margin of room?

2) I have very muscular, sloping shoulders. My traps extend completely to the tip of my shoulders. Is there a certain style or cut that might compliment build?

In general, with sloping shoulders, if you want an even shoulder line you have to build up the shoulder of the coat.

The other option is having things made for you.
