I think what you mean by "Neapolitan Shoulder" is where there are many little dimples at the sleevehead - a result of a large sleevehead being 'pleated' into a small armhole (arms eye?).
I would term the shoulder you posted as a "pagoda" shoulder, or "con rollino".

Interesting. These two Attolini coats have no rope at all: http://cgi.ebay.com/ws....me=WDVW http://cgi.ebay.com/ws....me=WDVW That's the Attolini look I'm used to. In any case, a lot of roping is contrary to the Neapolitan principle of softness. There should be at most a wafer thin pad and a little strand of wadding at the sleevehead. The pleats of the upper sleeve are worked around the wadding.
