MTM - What cannot be changed from the pattern?

So, as I understand, bespoke suits are made from scratch from a custom-designed pattern for the individual's body; whereas MTM suits start with a "pre-existing pattern", and then "adjustments" are made to the pattern in order to form it to the purchaser of the suit.

Things I KNOW can be adjusted on an MTM suit:
- Lapel width
- Jacket length
- Gorge / Button placement height

But can things like the below be adjusted?
- shoulder width (down to the 0.25")
- collar height(width) around the neck
- Arm-hole/bicep width

Essentially, I'm trying to pin down where exactly bespoke suits have the "advantage" in "adjustments" over MTM, as it specifically relates to the fit of the suit...
