Looks more casual then dressy, you will have a hard time finding anything close. With the buckles that low the look will be awkward, sort of like a soccer shoe with the flap that covers the strings. Good drawing though I think it gets your point across.

What's the rear buckle for? What's the point of having the straps almost touch the ground?

I foresee one of two possibilities. You commission a bespoke which runs you $3000.00 and change. Or Mark Nason comes out with that exact model within the next 6 months for $159.99. I give Nason 50/50 odds on this.

They also remind me of Red Wing Metatarsal Guards

I don't think you'll find anything mass-manufactured with that style because the design is not good. The front buckle will go bad quickly, being that low. Hanging outside the protection of the bottom surface of the shoe, it will snag on things, catch the elements, get crunched on debris just walking around. You mentioned riding horses, imagine what stirrups will do to that front, low buckle.

There is a reason just about every shoe keeps such design elements closer to the top of the shoe. They take less of a beating there.

If you are to accept a higher buckle position, then these Santoni double monks with a plain captoe might do the trick: Santoni Jadwin

pretty good. But, I'm a 12.5 E/EE. The general lack of 12.5s, as well as the added complication of a wide ball and narrow heel, plus the general rarity of double monks, and the odd fit between that shape foot and that style shoe, is really pushing me towards bespoke.

They're the "Cascade" model, by Hall & Marks, which is a Japanese shoe brand made by the Guild of Crafts.

I suspect that they would be quite expensive indeed - and they are most likely only obtainable in Japan.
