Lengthening surgeon's cuffs

Bought a nice Ring Jacket Meister-line navy hopsack sportscoat from Japanese Yahoo Auctions. It fits great except that the sleeves are too short, by about a quarter of an inch. They're surgeon's cuffs and to add to the dilemma, the distance from the last button to the cuff is already, for some reason, 1 3/4".

Is there any way to fix this? I'm not sure there is material to extend it from the sleeves, and even if there were, I'm worried that my tailor would mess up the nice spalla camica (I live in NYC and would go to a good tailor but still...). Is there a way to sew up the first couple of buttonholes and add below?

4.5 cm distance from the buttons to the end of the cuff is totally normal.
Letting out 0.6 cm would not be an issue. However if you think that would throw off the balance, you can ask your tailor to add an extra button below existing ones. It's going to look fine as long as you'll end up with no more than 5 buttons on a sleeve
