I'm thinking of buying some John smedley merino wool. Firstly how does it size up ? I have a 38" chest so what size should i buy ? It seems they do a fitted line and a classic line. I assume with the fitted stuff your not suppossed to wear anything under it. Is it acceptable to wear a sweater without anything under it ?

Yup. Do it. You'll look great.
Classic or fitted - wear it like a polo shirt.
Add a jacket or not.

A very cool look.

Smedley is good, but I think it used to be even better.

Chest measure of the knit should be 2-4 larger than your own. So you're probably a Small.

Absolutely.
Vintage Smedley is much sought after.

Smedley long sleeve polos are part of my look for winter here in Atlanta. They are durable, comfortable and look great. They are mostly from 90s and bought from Bergdorfs or in London at Harrods.

Highly recommended. Just use a gentle cycle if you wash any.

Is it acceptable? Maybe, if the body parts underneath don't show through. However, an undershirt will help the sweater last longer.
