Is it possible for the skin overlying the glans to become keratinized?

Your entire outer layer of skin is keratinized.

You can get a yeast infection which makes the skin itch, crack and sometimes bleed, it also can become tighter and not retract easily.

and to have the ensuing tumor press on the brain stem to the point of disinhibition of the speech center? It's rare, but I can think of one instance.

absolutely.
had a patient with an SCC of *********.
came in with lower limb oedema resembling heart failure at first glance, turned out to be an SCC of the glans that had been going on for a few weeks. turned out it'd metastasised to his abdo lymph glands and beyond. not nice.

last i saw of him was in the paliative care wards for chemo. not much chance unfortunately
