Inseam: crotch to ankle?

measuring inseam is from crotch to:

1) ankle level
2) below ankle
3) bottom of the foot (floor)

Also, where do you where your pants; above or below the hip bone?

Inseam is from the crotch (of your pants) to the cuff of your pants.
