Is there any high end custom t-shirt places? I want to make my own t-shirt, but from past experience either the material or fit is bad.

Is there any place with good quality materials and perhaps different fits that one can choose from?

Buy your own blank t-shirt from your favorite store and then go to your local t-shirt printing store. Cheaper and better.
