Help with Foot Problems: Bespoke/Custom Shoes in Chicago

I have what is called Hallux Rigidus, which is arthritis of the big toe. It causes pain and a bone spur that resembles a bunion. I get great relief wearing rocker sole shoes (MBT) but they are ugly and inappropriate for dress wear. I can get minor relief by inserting a carbon fiber plate in my dress shoes but that only works if the shoe has a removable insole that permits me to put the carbon fiber under the insole. Decent dress shoes don't have removable insoles so I tried putting in the plate plus a cushion insole but that throws off the fit like crazy and renders shoes unwearable.

So, I need to explore getting custom shoes. Essentially, I am looking for an Alden quality shoe that has a removable insole and a bit of extra depth so I can place a carbon fiber plate under the insole. Unfortunately, the cobblers I've spoken to claim that they cannot modify RTW shoes to accomodate this. All have said I need to find a custom shoemaker

Can anybody recommend a custom shoemaker in Chicago? Or suggest alternatives to assist me in resolving this problem.

Have you tried AE 8 last? Apparently it's intended to be worn with an insole.

Thanks guys. The G&G suggestion - while excellent - is probably too much money for me. I looked at the AE 8 last and it wasn't bad but I'd much prefer leather soles. thanks again
