Big English Premier League fan here and I've noticed this hooded cardigan worn recently at a few matches by Pep Guardiola (attached).

Does anyone know the label/model and where I can get it from?

sorry, no. but i'd say yoox is your best bet for something similar.

Try Brunello Cucinelli and Esemplare -- both available on Yoox, both make plenty of knit outerwear, sometimes with nylon (occasionally padded / down-filled) interior.

Looks like something Esemplare would make, I remember NMWA stocked something similar a few years back. No clue what happened to the brand.

Here's one on ebay for 200 bucks used which seems steep!
