Help selecting MTM / custom shirts

Great forum, been lurking for a while and finally decided to jump in.

I have a dozen or so off the rack Borrelli, Brioni, and other rather high end shirts that I really do love and appreciate. The only problem is that at 5'10" and 130lbs, I'm rather skinny, and few shirts fit me the way that I would like. The more I read, the more it sounds like a MTM or fully custom made shirt may be the way to go. I was considering going into my local Borrelli boutique (Boston MA), but after reading how it may be possible to find equal or better quality for a small price tag, I thought I'd get some opinions on just what to do. I don't mind spending the money (<$400 / shirt) but I want to maximize my return.

Any recommendations of places to try, various options, etc, would be appreciated. Thanks.

If you go down to NY, I would check out, in this order (and that price range)

1: CEGO (for that much cash you can have them made in house, I imagine)
2: Geneva
