Fixing Armholes on a Shirt

If a tailor has made a shirt with armholes that are too small, can the problem be resolved by simply remaking the sleeves, or does the entire shirt need to be remade?

should be all right if he just changes the sleeves (you need more fabric) and cuts larger armholes.

I would be intersted in the opposite:

Is it poosible to narrow the armholes on a RTW shirt?
