I would take the waist in by 1" total (front & back so half measure would be .5") and the arm holes by the appropriate amount. Looks like 1" total from my computer, but thats...from your pictures. Pinch it and see how much you want to lose.

They won't--why not? Admittedly I've never made a shirt, but I've taken a few apart and making the placket an arbitrary size seems like a trivial thing to do.

Do you mean they just can't alter it once it's made?

most factories attach the center placket via a chain stitch machine. it is a big deal to change the settings on this machine. so they are usually standardized.
the fronts can be turned by hand and adjusted, but i doubt the factory wants to do that with out an upcharge. It takes a little longer to match stripes.

the collar looks a tad stiff for a button down, hence the fusing question.
not a bad thing.. just something i noticed.

you can have different sized cuffs to accommodate a watch.

depth of armholes look ok.
