Something like this? But with a medallion? If you Google a bit you might find someone that carries it in stock. But I'm not sure, it's not a very common style, usually it will be a quarter brogue. Of course, the MTO route is available as well.

804 has one single width if I remember correctly.

Yes 804 is available in wider width but I am not sure if it is slightly wider width or double EE. My understanding is that EB offer following width from narrow to wide; E , F (standard width), G, H, I do not now if 804 last comes in either G or H. It is best to check with retailer.
I have EB 946 last in G width and that corresponds to American E width in opinion.

Yes there is plain toe cap toe from EB, please see here https://us.skoaktiebolaget.com/collections/enzo-bonafe/products/enzo-bonafe-dress-shoes-3863

Yes! Thunder March's link is what I was trying to describe. No medallion and the only brougueing (is that a real word?) is a single strip at the base of a plain captoe. None anywhere else. Like the G&G "Cambridge."

I have an EB black stitchcap on the 804 from a GMTO with Skoke, my only EB to date. The fit tolerates my high instep better that any shoe I've owned but I wish I could magically make it just a bit wider.

Unless the 946 is wider, I might want to MTO an 804 in G. Thanks to all for the input.

Yes, 804 is available in G width, and I'm positive they'll made a punch cap Oxford for you.

If anyone is down for a dark brown unlined chukka (like the Alden unlined chukka, but by EB), or a jumper boot in the horween arlington grain, let me know-I'd be down for a GMTO in both
