Looks like a Banbury with double leather sole and a stitched heel counter akin to that on the Halifax.

As to color, EG does not have a color called "chocolate" though I like it. It looks pretty close to what the London shop calls "coffee".

Can't tell the last, though I'd guess a 202 or 606.

Let me recant, having taken a look at a catalog, it is probably a Warwick - the heel counter and a higher set of lacings being the main differences to the Banbury that I can see.
