Does it affect the height of the heel, or just the thickness of the sole?
I'm not 100% sure that there's a general rule that applies to all shoe-makers. I've seen some double-soled shoes where the double sole part appears to exist only in the front sole, not the heel (the extra "double" piece of leather tapers off near the heel). On the other hand, it wouldn't surprise me at all to find that a maker such as Alden makes a double=soled shoe where the extra leather continues across the entire length.
