Just starting to read through this thread as I am a fan of a double breasted suit or jacket but wtf is that? It looks like your tailor just put the button in the wrong place and the shoulders are far too narrow, they’re bursting at the seams.

Also good on you for not being ashamed to wear orthopedic shoes.

I would suggest a much larger brim Panama hat, just in proportion to your jacket lapels.

I would agree that the shoulders could be slightly wider (particularly the right one), and it might stylistically improve the clean and straight lines. But I like the button placement and everything else.

Yes, a slightly boxy waist. DBs should always have a little bit of pull on the button point. It should definitely be taken in a bit but even so, it still looks very good.
