Might be easier to make it yourself. You trace the pattern with paper around the outsole, subtract 5mm from all edges, then iteratively trim the paper until you get a good fit inside (just press it against the edges and the creases provide the contour to cut). That's your pattern.In my browsing here I've seen several drawings of the various last shapes available from Carmina. Is this something that Carmina provides? Asking because I'm looking to have a custom pair of leather insoles made for my 9.5 Detroit boots.

Afterwards, trace on to vegtan leather of appropriate thickness. Rasp and glue heel in place if needed (see image2) - use vegtan shoulder, or neoprene spongey foam if you want a softer feel. I made these to take up the volume in my jodhpurs until they conformed to my foot and stopped slipping in the heel:

If you don't have the tools to do a nice job on the leather, make your own test run in cardboard first and can adjust the thickness until you get it right so you commission something appropriate.
