Black Dress Shirt - Where to Buy?

This is not a "should I" or "what do I pair it with" question. I saw that thread here.

I am looking for a quality jet black dress shirt, spread collar, French or barrel cuff, 100% cotton, slim fit (Brooks Brothers Extra Slim Fit is a perfect fit in 15.5/32). Quality dye preferable so it doesn't fade after the first wash (Woolite black, cold wash, hang dried).

Ideas? I'm hoping to hear from folks who actually own one.

Since you have something very specific in mind, you might think about doing MTM. From all reports, Luxire does a fine job, for a pretty good price: http://custom.luxire.com/search?q=BLACK+SHIRT. Looks like in 100 cotton, you can get a heavier weight twill or a dual tone black (double sided?) evening shirt.

I'm also looking for a black dress shirt. Preferably something in flannel or oxford cloth in a more casual fabric. Something with an inch of extra sleeve would be ideal.
