I thought Derek Rose may have a bespoke option but I can't find it on their site anywhere...

I was inspired by your question and search for it, people on London lounge were talking about having their shirt makers make them bespoke pyjamas. This may be a bridge too far, in fact it may be several bridges too far in my view.
Are you looking for a better fit and custom details in what you wear to sleep in?

sure why not.
I have even made nightshirts!

Piping on the front edge and collar, etc, can add quite a bit as most shirtmakers would have to make this by hand, where companies like derek rose have machines to produce the piping.

+1. My former shirtmaker did it, my latest shirtmaker also does it. I fact, I can't think of any local shirtmaker who doesn't.

I would opt for mor sturdy fabrics with a bit of volume. Stay away from super-light-look-at-me-I'm-so-shiny-and-beautiful fabrics as they will wear out more quickly.

Especially if the only one to see the fabric of your pajama is supposed to remove it anyway
