Bespoke shirt wear considering MTM shirts

My RTW work shirts will need to be replaced soon and am considering both mtm and bespoke shirts. I have shirts from Sean O'Flynn and after a bit of nip-and-tuck on the first shirt, he has the fit spot on.
However, taking cost into account, I am willing to sacrifice very minor fit issues over cost for work shirts; although I have a feeling I could be disappointed with mtm shirts and regret not spending the extra money (especially as I do not have mtm shirts).

For those who wear mtm and bespoke shirts, do you find any major differences between the two?
